Directions to our office - we are across from Checkers & Chubby Checkers, in the old State School property off Route 202:

From the center of Belchertown: At the lights in the center of town (near Town Hall), proceed South on Route 202 (towards Granby). At the flashing yellow light, take a right onto Front Street and an immediate right onto Bridge Street (Hulmes Transportation is on the corner). We are the last grey building on your left.

From Granby, Ludlow, South Hadley: Proceed North on Route 202 (towards center of Belchertown). After you pass the Police Station and the schools, you will come to a flashing yellow light. Take a left onto Front Street and an immediate right onto Bridge Street (Hulmes Transportation is on the corner). We are the last grey building on your left.

From Palmer/Three Rivers: Proceed on Route 181 into the center of Belchertown. Turn left down Route 202 South (towards Granby). At the flashing yellow light, take a right onto Front Street and an immediate right onto Bridge Street (Hulmes Transportation is on the corner). We are the last grey building on your left.

Fax or deliver a dated and signed release form to the practice that has your records. It's a good idea to contact your old provider to ensure that your forms have been received and are being processed.

When you arrive at our office for your first appointment, you will be asked to complete some paperwork. To expedite your check in process, you may download the registration paperwork below, complete it and bring it with you to your first appointment.
